should already have filled out part one.

Our database requires non-null entries of the following information
for all who attend.

address
phone number

In addition, our database will happily ingest the following information
for each person where it is relevant.

e-mail address
personal WWW URL
fuel source
number and sizes of VirtuAle T-shirts desired
number of VirtuAle hanks desired
This is not a secure form; we do not accept the roster via WWW.
If you feel secure sending it we will accept the roster via email;
see the contacts below.
Otherwise please submit the team roster on paper via USPS along with
your check. Don't forget to go on to part three.
